Those with Narcissistic Personality Disorder may be quite charming and successful individuals, but also have difficulty relating to others closely. Those with Narcissistic Personality Disorder tend to see things in 'all or none' terms and view themselves and others as either 'all good' or 'all bad'. Often they cannot admit to even a minor mistake as strong feelings of worthlessness are felt if they do. Frequently they seek constant approval and admiration and expect to be treated with favorable, 'special' treatment. They lack empathy and seem to be unaware of the effect of their behaviors on others. NPD's are especially sensitive to criticism and defect and may become quite angry at the smallest slight they perceive from others. They frequently maintain their ego defenses when they are doing well or are successful, but under stress, may become depressed or even suicidal. Other may become 'suffering martyrs' when they are not 'conquering heroes'. These individuals are helped when they are able to view themselves as worthy people who make mistakes. Treatment is often considered to be threatening by the client, due to strong, protective ego defenses in place, therefore, it is recommend experienced therapists are sought who are able to help the client deal effectively with these issues.
